CUSTOMER SURVEY NOW OPEN (9/21/2016)
To our valued customers:
It is time for the Utah Division of Oil, Gas and Mining's (OGM) annual customer satisfaction survey to determine if we are meeting your needs and what we can do to improve our services.
This short, six question survey will help us identify the areas where we excel and those that could use improvement. To access the survey, please visit https://www.surveymonkey.com/r/96DMYK5.
This survey will run September 21 to October 19. We welcome and encourage your feedback.

NOTE: Operators may now amend existing reports previously entered into the system. Original reports should be amended when corrections are needed, rather than creating new reports for the same event.
