NOTE: Operators may now amend existing reports previously entered into the system. Original reports should be amended when corrections are needed, rather than creating new reports for the same event.

This program allows oil and gas operators to electronically submit Applications for Permit to Drill (APD), Sundry Notices, and Workover Tax Credit applications to the Utah Division of Oil, Gas and Mining.
Operators can now submit their WELL COMPLETION REPORTS (Form 8) and associated attachments by uploading a single pdf document through ePermit. A "Completion Report" button is located on the Main Menu. This option replaces the need for submitting paper copies of this report (hard copies of all required well logs must still be mailed to the division at this time).
A registration form (below) must be submitted prior to using this system.

A company must register to use the ePermit system by completing this form and mailing it to the Utah Division of Oil, Gas and Mining prior to electronically submitting Applications for Permit to Drill (APD), Sundry Notices, or Workover Tax Credits. The original, notarized form must be submitted. Copies will not be accepted.
